Media Consent Form
ACADEMIC & WELLNESS COACHING LLC MEDIA RELEASE
MEDIA RELEASE AGREEMENT
This Media Release Agreement (the "Agreement") is entered into, by and between Academic & Wellness Coaching LLC, a California Limited Liability Company, doing business as AWC ("AWC"), and the undersigned participant ("Participant").
WHEREAS, AWC conducts workshops and events both virtually and in person, and may engage in various media activities including but not limited to photography, video recording, interviews, articles, and artwork creation during these workshops and events;
WHEREAS, Participant wishes to participate in the workshops and events organized by AWC, and hereby consents to the use of their likeness, voice, and artwork in any and all media, including, but not limited to, photographs, videos, interviews, articles, and artwork created during the workshops and events.
NOW, THEREFORE, in consideration of the mutual covenants contained herein, it is agreed as follows:
1. Media Release:
Participant hereby grants Academic & Wellness Coaching LLC (AWC) the irrevocable right and permission to use, reproduce, publish, distribute, and broadcast Participant's name, likeness, voice, artwork, and any other material or representation of Participant's identity (collectively, the "Media"), in any and all media now known or hereafter devised, worldwide, in perpetuity, for advertising, promotional, educational, and other purposes without any further approval or compensation.
2. Waiver and Release:
Participant hereby waives, releases, and discharges AWC, its officers, employees, agents, and representatives, from any and all claims, demands, damages, actions, or liability of any nature whatsoever, whether known or unknown, arising out of or in connection with the use of the Media as contemplated herein.
3. Agreement Confirmation:
By checking the box indicating their agreement to these terms, the Participant confirms their full understanding and acceptance of the terms and conditions outlined in this Agreement.
4. Governing Law:
This Agreement shall be governed by and construed in accordance with the laws of the state of California, without regard to its conflicts of laws principles.
5. Entire Agreement:
This Agreement constitutes the entire agreement between the parties and supersedes all prior and contemporaneous agreements, understandings, and representations, whether oral or written.
