Media Release

I hereby waive, release, and discharge Academic & Wellness Coaching (hereinafter referred to as “AWC”) from any and all claims, demands, damages, or liability of any nature whatsoever relating to this publicity release.

 

Media includes:

• Art Work. Any work, in any medium, created by me/my child or represented as my creation. 

• Photograph & Video Recording. Any still or digital photography, motion picture, Zoom Session Recordings, or any other means of recording or reproducing images including social media, that contains my/my child’s likeness, either taken by me or by AWC. 

• Interview or Article about me/my child (either written by AWC, a third party or me).

I understand that AWC may use the Media in advertisements, social media, newsletters or other materials, in connection with education and training, advocacy, promoting and publicizing AWC in any manner or form. 

 

AWC shall be without liability to me for any distortion or illusionary effect resulting from the use of the Media above-mentioned materials.

 

I understand that this Media may be seen by the general public and once the materials have gone into production, it may no longer be possible to retrieve them.

I understand that I shall not be compensated or paid for the use of the Media and that I will be able to review and approve the video before distribution.

I represent that I am over the age of eighteen (18) years and fully understand the contents of this release.*
